Community mental health care encompasses a wide variety of program and services designed to meet local needs. These programs are delivered primarily by community agencies and sometimes through hospitals or health clinics. The majority of programs provided by community-based agencies are designed to serve the most vulnerable and most severely mentally ill. Outreach offices are located in Petrolia, Forest and Kettle Point.

Pathways Health Centre for Children is a family-centred community agency that serves Sarnia-Lambton children and youth with physical, developmental and communication needs and their families, as well as adults for some specialized services. The Centre provides a continuum of rehabilitation and support services in the community as well as an on-site integrated preschool.

The Community Care Access Centre (CCAC) was established by the Ministry of Health and Long-Term Care to provide access to government-funded home and community services and long-term care homes.
